Immediate placement
Teeth in a day —
when the bone says yes.
Extraction, implant placement and a fixed provisional in a single visit. It is a real protocol with real limits, and the honest version of this page is mostly about the limits.
Why patients come here
- Practice limited to prosthodontics and oral implantology
- Every case CBCT-scanned and fully guided
- IV conscious sedation available in-office
- A written, itemized plan before anything is scheduled
What decides it
Primary stability, measured on the day.
An implant can be loaded immediately only if it is held firmly enough by the bone at the moment it is placed. That is measured directly — insertion torque, and where indicated a resonance frequency reading — and the number either clears the threshold or it does not.
Bone quality is the variable that decides it, and it is the subject of Dr. Resnik's master's thesis: the primary stability of tapered versus parallel-walled implants in poor-quality bone. In soft bone, a tapered design and an under-prepared site can produce the stability that a standard protocol will not.
If the reading is short, the implant still goes in and still integrates — it simply heals unloaded for a few months while you wear a well-fitted temporary. That is a scheduling change, not a failure, and it protects the result.
Candidacy
Good candidates, and the ones we stage.
Favors same day
Intact socket walls, dense bone, no active infection at the site, a bite that can be relieved so the provisional is not loaded in function.
Favors staging
Soft bone, a blown-out buccal plate, active infection, heavy grinding, or a smoking habit that meaningfully affects early healing.
Full-arch immediates
Often more predictable than single-tooth immediates, because four to six implants share the load and can be splinted together on day one.
Front teeth
The aesthetic stakes are highest and the bone is thinnest. Frequently done, but with a provisional deliberately taken out of the bite.
Back teeth
Molar sockets are wide and irregular; an immediate implant often needs grafting around it, which can change the timing.
Second opinions
If you have been promised same-day teeth elsewhere without a CBCT scan, that is worth a second look before you commit.
The day
What to expect, hour by hour.
You arrive having not eaten if you are being sedated, and with someone to drive you home. Failing teeth are removed as atraumatically as possible — preserving the socket walls is half the work — and the sites are debrided.
Implants are placed through the surgical guide, stability is measured, and if the numbers clear the threshold the provisional is fitted and adjusted out of heavy contact. You leave with teeth, written aftercare, medication and a direct number.
Living with a provisional
- Soft diet for six to eight weeks — this is the rule that most affects the outcome
- No biting into anything with the front teeth while integration is happening
- Swelling peaks around day three; bruising is common and resolves
- The provisional is deliberately slightly out of the bite, so it may feel like it is not touching
Start with a scan and a straight answer.
A consultation includes an examination, a 3D scan and a written, itemized plan. If implants are not the right treatment for you, we will say so and tell you what is.
