Privacy notice
What this site collects,
and what it does not.
Short version: the consultation form collects the contact details you type into it, and nothing else about you is gathered here.
Why patients come here
- Practice limited to prosthodontics and oral implantology
- Every case CBCT-scanned and fully guided
- IV conscious sedation available in-office
- A written, itemized plan before anything is scheduled
The form
Information you choose to give us.
The consultation request form asks for your name, phone number, email address, what you are looking to replace, and anything you choose to add in the message field. It is used to contact you about an appointment and for no other purpose.
Please do not enter detailed medical history, diagnoses, medications or insurance numbers into the form. A website form is not a secure channel for that information, and none of it is needed to book a consultation — it is gathered properly at your visit.
Where it goes
Form submissions are delivered to the practice and handled by the practice team. They are not sold, rented or shared with advertisers, and they are not used to build a marketing profile of you.
Protected health information
Information you share during treatment is protected health information and is governed by the practice’s HIPAA Notice of Privacy Practices, which is provided to every patient and is separate from this website notice. Ask at the front desk for a copy.
The website itself
Cookies, analytics and third parties.
This site sets no advertising cookies and runs no cross-site tracking of its own.
Video segments on the site are hosted by YouTube and load only when you press play. Once you do, YouTube may set its own cookies under its own privacy policy. Nothing loads from YouTube until you choose to watch.
Links to third-party profiles — review platforms, directories, publishers — lead to sites this practice does not control and whose privacy practices are their own.
Children
This website is intended for adults arranging their own care or the care of someone in their family. It is not directed at children and does not knowingly collect information from them.
Changes and questions
If this notice changes, the revised version is posted here. To ask what the practice holds about you, to correct it, or to ask that it be deleted, call the office — the number is at the top of every page.
